I RESERVE THE RIGHT TO NOT PRESCRIBE XANAX, AND TO DISCHARGE ANY PATIENT WHO MISREPRESENTS THEIR PAST HISTORY IN ORDER TO OBTAIN CONTROLLED SUBSTANCES.
Insurance
In order to make sure that I am able to provide the best possible care, I
have chosen to become an out-of-network provider for most major insurances.
This means that I do not take insurance as direct payment at this time, but
will be happy to provide you with the necessary forms so that you can file
for reimbursement from insurance yourself. In this way, you and I are in
charge of your care, instead of an insurance company dictating how often you
can be seen or what treatment you can receive. Your insurance will still pay
for your medications and may reimburse up to 50 percent of your visit costs
after an initial deductible. The reimbursement rate varies greatly and
depends on your individual policy coverage for out-of-network providers.
